Patient-Centered Medical Home ?

Coastal Pediatric Care is recognized by NCQA as a patient-centered medical home.

A Patient-Centered Medical Home has five core functions:

Comprehensive Care

We provide preventative, acute, and chronic care to meet the majority of our patients’ medical needs with a team of physicians, nurses, and staff.

Patient-centered

We partner with families and patients to provide health care for the child as whole person, understanding everyone has different needs based on culture, beliefs, and preferences.

Coordinated Care

We coordinate care with specialists, hospitals, home health care, and community services.

Accessible Services

We are available 24/7 by phone (727-525-2161) for urgent questions. For a true medical emergency, dial 911.

We offer convenient same-day and next-day appointments, after-hours phone access and extended hours including Saturdays.
